Why is a dedicated Pharmacy service required?
It is well known that we are an ageing population. This fact, together with taking into consideration other identifiable vulnerable groups, we know that we have many people with increased disease states receive who recieve a higher number of prescribed medications – a term we refer to as polypharmacy.
Low medication adherence or low compliance is a known and real complex problem in polypharmacy in vulnerable patient groups (Drugs and Ageing – April 2014).
Polypharmacy in these vulerable groups leads to over-ordering. The patient feels confused due to lack of control which, in turn can easily lead to over compliance, incomplete compliance, or even non compliance and drug wastage.
Non or little medication compliance, whether that is intended or not, inevitably means that the disease state of the patient will be adversely affected. And when this happens, the patient will need medical attention, often urgent which can result in hospital admissions.
Our full medication management service will reduce stress upon the patient, carer, discharge teams and homecare support agencies. Having a robust system in place increases patient adherence to medication, thus achieving better health outcomes, allowing the patient to maintain their own health and remain at home for longer.
What PDO offers – Our Medication Management Service
PDO Pharmacy provides a solution to individuals and families who need care and support in managing their medication. Our primary aim is to provide medication compliance aids* direct to the patient’s home. Our service is designed to fit individual needs thus facilitating a persons choice, independence, privacy and dignity. We recognise the need to support people at home is not only beneficial to the patient’s health and well being but our service will play a integral role in supporting adult and young Carers, Home Care Support agencies and family members. We pride ourselves with having NO waiting list, a 9am to 9pm daily after care support line and are much more that a prescription collection and delivery service. After referral, we will deal with all aspects of your medication from liaising with the GP to delivery to your door. We will be a single first port of call for information and advice on any aspect of your medication.
*Medication Compliance Aids (Dosette boxes,pill organisers, calender blister packs) are individualised boxes containing medications organised into compartments by day and time. They are primarily designed to help a person remember to take their medication at the right time of day. They can especially be useful when other methods of promoting compliance are not working.
We serve those patients referred to us to have their tablets and capsules professionally placed into our fully disposable weekly medication compliance aids. We co-ordinate the entire repeat prescription process with the GP surgery ensuring that all medications coincide, prescriptions are ordered on time and any changes to medications are dealt with as soon as possible.
As we appreciate the importance and urgency of every referral we will NEVER place anyone on a “waiting list”
PDO Pharmacy understands that you may find it difficult to collect medication therefore we offer a rapid, discreet and reliable delivery service free of charge. This is extended to where a client would like us to manage the ordering, dispensing and delivery of their prescription but does not require a compliance aid or that a client has simply ran out of a regular medicine that is not placed inside a compliance aid e.g inhaler, eye drops.
